Origins of DFTBA

Now, I know the video in which John first mentions the initialism, but it wasn't the first time it had been mentioned. John's friend Katie had it written on her arm, and I have often read that it was popularised by the vlogbrothers. Does anybody know when it was first mentioned? Was it a less known vlogger or something like that?
